Amend Registration Details

This tool will allow you to modify basic information if you have made a mistake on your test registration. This information will be printed on your test certificate.

You can edit:

• Patient Name
• Sex
• Address
• Date of Birth
• Passport ID
• Email
• Phone Number

Upon completion, you will be notified by email of the changes.

You can only make changes to your registration ONCE, after this the record will not show in this tool.

You can only make changes to your registration before you take your sample or until a sample ID is registered to your record.
